We have a great opportunity for a registered Health or Social Care professional.

Your role and responsibilities

You will hold a key role to support the development of integrated working across Health and Social Care through the development and management of the Integrated Neighbourhood Teams, including the associated Connect community.

You will have joint accountability to Adult Social Care in Ipswich and East Suffolk and to East Suffolk and North East Essex Foundation Trust (ESNEFT).

The main purpose of your role will be:

  • to develop, directly manage and lead the Integrated Neighbourhood Team, reporting to Senior Management teams across both organisations
  • to lead on specific clinical/care governance activity, including the setting and monitoring of standards and performance, responding to complaints, staff support, learning, development, supervision, and the implementation of policy and clinical guidelines
  • to oversee implementation of the health and care strategy at the neighbourhood level working closely with all partners including primary care, acute care, specialist services, mental health services, housing, the voluntary sector, and care providers
  • to build relationships within the Connect community.

  • professional registration
  • degree level qualification in own specific professional field or equivalent relevant experience.

The team

Ipswich and East Suffolk Adult Social Care have 8 Integrated Neighbourhood Teams bringing Health and Social Care Services together to provide a single coordinated care response that is underpinned by prevention, self-care, early intervention, reablement and rehabilitation rather than longer-term treatment and lifelong service dependency.
